				<content:encoded><![CDATA[<p>While municipalities often boast the merits of growing food with sewage sludge (also known as biosolids), the FDA has just done three studies showing a problem with our food being contaminated with with perfluoroalkyl and polyfluoroalkyl, some with very high levels.</p>
<p>Colin O’Neil &amp; David Andrews from EWG wrote last week in <em>PFAS Central</em> that &#8220;A recent investigation by the Food and Drug Administration found toxic per- and poly- fluoroalkyl substances, or PFAS, in food, including meat, seafood and dairy products; sweet potatoes; pineapples; leafy greens, and chocolate cake with icing.&#8221; [1].</p>
<p>Where are these PFAS coming from and does the organic standard protect us from these chemicals?</p>
<h2>Where do PFAS come from?</h2>
<p>Exposure from PFAS in humans are thought to originate from <strong>agricultural products grown with sewage sludge</strong>, also called <strong><a href="https://en.wikipedia.org/wiki/Biosolids" target="_blank">biosolids</a>, </strong>from <strong>food packaging</strong> and from <strong>drinking water</strong> polluted with these chemicals.</p>
<p>&lt;&lt; The FDA detected PFOS in approximately half of the meat and seafood products; PFPeA in chocolate milk and high levels in chocolate cake with icing; PFBA in pineapple; and PFHxS in sweet potato. &gt;&gt; [2]</p>
<p>They are also used in various commercial products. The EPA has identified more than <strong>600 chemically similar PFAS compounds</strong> in active commercial use.</p>
<p>&lt;&lt;Almost half of the seven million tons of sewage sludge generated in the U.S. every year are applied to land, including on farm fields. &gt;&gt;  [1].</p>
<p>Even in Canada municipalities recycle sewage sludge into agricultural products [ref https://www.ledroit.com/actualites/boues-de-la-station-depuration-de-granby-de-dechets-a-engrais-e24a30f2c4b2beb9769b9538228d6e10].</p>
<p>Not all contamination came from agriculture &#8212; contaminated chocolate icing was thought to originate from another source: &lt;&lt; 17,640 parts per trillion (ppt) of perfluoro-n-pentanoic acid (PFPeA) in chocolate cake with icing. These levels suggest that the cake was contaminated from the intentional use of the chemical to greaseproof paper that contacted the cake rather than from an environmental source. &gt;&gt; [ref http://blogs.edf.org/health/2019/06/03/fda-high-levels-pfas-chocolate-cake/]</p>
<h2>What are PFAS?</h2>
<p>These are molecules containing multiple (13 to 17) <strong>fluorine</strong> atoms along with ~10 other basic atoms like carbon, oxygen and hydrogen. Unfortunately these <a href="https://www.atsdr.cdc.gov/pfc/docs/pfas_fact_sheet.pdf" target="_blank">man-made</a> chemicals do not break down in nature; they are bio-resistant[3][4] and are associated with a long list of health issues.</p>
<ul>
<li>PFAS is the whole group of per- and poly- fluoroalkyl substances. It includes man-made chemicals like PFOA, PFOS, GenX, and many others [ref https://www.epa.gov/pfas/basic-information-pfas].</li>
<li>FFOS is a short for <a href="http://Perfluorooctanesulfonic acid" target="_blank">Perfluorooctanesulfonic acid</a>. They are often used as a <strong>surfactant</strong>, or more precisely fluorosurfactant.</li>
<li>PFOA are also known as <a href="https://en.wikipedia.org/wiki/Perfluorooctanoic_acid" target="_blank">Perfluorooctanoic acid</a> and also affectionately known as C8. In french they are called APFO (acide perfluorooctanoïque). They are also <strong>fluorosurfactants</strong>. They are used among other things in the fabrication of cookware and strain-resistant carpets, microwave popcorn bags, some brands of <strong>dental-floss</strong>, floor wax, fire fighting foam and many more items. Those are now banned in US and Canada, although it is not mentionned with what it has been replaced [ref <a href="http://www.scientifique-en-chef.gouv.qc.ca/impacts/ddr_poeles-antiadhesives-faut-il-se-mefier-de-lapfo/" target="_blank" rel="noopener">http://www.scientifique-en-chef.gouv.qc.ca/impacts/ddr_poeles-antiadhesives-faut-il-se-mefier-de-lapfo/</a>]</li>
</ul>
<p>While more anti-stick cookware are now manufactured without PFOA, as previously said, the EPA has identified more than 600 chemically similar PFAS compounds in active commercial use but has set no legal limits or health advisories for those chemicals in water, air or consumer products[2]. It might be just like when <a href="https://www.scientificamerican.com/article/bpa-free-plastic-containers-may-be-just-as-hazardous/" target="_blank">they replaced BPA with BPS</a> and now label things as &#8220;BPA free&#8221;.</p>
<h2>What are the Health Risks of PFAS</h2>
<p>PFAS have been <strong>associated</strong> with cancer, reproductive harm, developmental harm, high cholesterol, damage to the immune system, hormone disruption, and liver and kidney damage. A top U.S. toxicologist now goes beyond just association and mention causal relationship with diseases [ref <a href="https://theintercept.com/2019/10/24/pfas-toxicologist/" target="_blank" rel="noopener">https://theintercept.com/2019/10/24/pfas-toxicologist/</a>]. &lt;&lt; In my mind, PFAS cause health effects because you have the same kind of effects reported in multiple studies in multiple populations &gt;&gt;.</p>

<p>Some scientists who work with biosolids (sewage sludge), with all their wisdom and lack of conflict of interests, mention that it is safe and useful.</p>
<p>&lt;&lt; Scientists say that the evidence overwhelmingly suggests that sludge is safe and useful. &gt;&gt; [7].</p>
<p>Recall when they found PFOA (aka C8) in water near DuPont factory. DuPont mentioned in a letter to residents that C8 was found in public water but according to their standards it wasn&#8217;t harmful [3].</p>
<h3>Immune Effects</h3>
<p>If you take time to read the <a href="https://www.atsdr.cdc.gov/ToxProfiles/tp200-c2.pdf" target="_blank">chapter 2 of the ATSDR Toxicological Profile for Perfluoroalkyls document</a>, you will see that perfluoroalyls have been shown to decrease antibody response to vaccines. One might pose the hypothesis that PFAS are causing an increase in vaccine-preventable diseases worldwide.</p>
<p>&lt;&lt; Immune effects. Evidence is suggestive of a link between serum PFOA, PFOS, PFHxS, and PFDeA levels and decreased antibody responses to vaccines. A possible link between serum PFOA levels and increased risk of asthma diagnosis has also been found &gt;&gt; [ref https://www.atsdr.cdc.gov/ToxProfiles/tp200-c2.pdf]</p>
<p>We are looking forward to look at the result of this <a href="https://www.circleofblue.org/2021/world/federal-agencies-plan-to-investigate-links-between-pfas-exposure-and-viral-illness/" target="_blank">larger study</a>.</p>
<h2>Does Organic Agriculture Allows Sewage Sludge?</h2>
<p>Luckily if you have been eating organic certified food, sewage sludge is not only prohibited but frowned upon by organic farmers.</p>
<p>&lt;&lt; In addition, USDA organic standards require that food <strong>cannot be grown</strong> in sewage sludge. &gt;&gt; [8]</p>
<p>&lt;&lt; Synthetic fertilizers, <strong>sewage sludge</strong>, irradiation, and genetic engineering <strong>may not be used</strong>. &gt;&gt; [9]</p>
<p>&lt;&lt; [Organic farmers] resolutely oppose allowing its use under the final organic standards rule. &gt;&gt; [10]</p>
<p>In 1997 the U.S. Department of Agriculture (USDA) and the Environmental Protection Agency tried to include a proposition to allow “high quality biosolids” (i.e., sewage sludge) to be used in organic food production [10]. Fortunately it did not pass and today the USDA organic standard still rejects sewage. Note that other countries have their own organic regulations, that often surpasses in safety the United State one.</p>
<h2>Food for Thoughts</h2>
<p>One problem with organic agriculture is that large agro-companies are often organic producers themselves. Some are actively lobbying to dilute the organic standard, and at the same time when they succeed, other influential players &#8211; maybe even the same shareholders &#8211; uses these deregulation to &#8220;expose&#8221; that the organic standard does not protect consumers better, failing to mention how it was lobbying that caused the USDA Organic Standard to have these loopholes in the first place.</p>

<h2>About MCL injuries</h2>
<ul>
<li>One of the most common type of ligamentous knee injury (40%)[1] and account for about 8% of all knee injuries[2]</li>
<li>Studies showed that the most two common sites of superficials MCL (sMCL) injuries were at the endpoints[3] :
<ul>
<li>the femoral attachment or;</li>
<li>the tibial attachment.</li>
</ul>
</li>
<li>Deep MCL and posterior oblique ligament (POL) injuries are also possible (not shown in picture above).</li>
</ul>
<h2>What to do if you have a MCL injury</h2>
<ul>
<li>Consult a sport physiotherapist ideally in a well reputed sport clinic.</li>
<li>Consult an orthopedist; only them are allowed to prescribe MRI and microsurgery inspection.</li>
<li>Depending on the grade, the knee might need to be protected with a short-hinged brace for 3 to 6 weeks.</li>
<li>Your physio or ortho will give you appropriate exercises.</li>
<li>Maintaining good muscle tone during walking and exercising is important to stabilize the knee &#8211; as opposed to letting your weight loose on your leg without tonus. (source: personal experience)</li>
<li>Wearing shoes that are somewhat slippery will reduce stress on the knee when doing sudden movements. If your shoes has too much grip with the floor, it can result in a more intensive  forces on your knees. This does not apply when outdoor with ice where safety is #1. (source: personal experience)</li>
<li>When turning, avoid using the tip of your foot which will create a lever effect and increase the torque (moment-force) on the knee; rotate using the heal instead. (source: personal experience)</li>
<li>Avoid taking the stairs and hold up the ramp if you must to stabilize. By stabilizing with your hands on the handrail you reduce stabilization requirements on your MCL.</li>

<h2>Grading</h2>
<p>If you hear various doctors you will notice that two different classification of grading are present, the <em>American Medical Association (AMA) Classification</em> and the <em>Hughston Modification of the AMA Classification.</em> The latest classification (Hughston) is not only based on joint laxity but also severity of tenderness and quality of the endpoint. Some practitioners still only use the laxity to grade the injury which represents the earlier AMA classification dating 1966.</p>

<h2>Recovery and Rehabilitation</h2>
<ul>
<li>Warm up before doing exercises</li>
<li>When you start sweating you are warmed up</li>
<li>Gently stretch all muscle groups after warming up</li>
<li>Do not injure again during the recovery period. If you must do high weight carrying and climbing stairs during your work, ask your doctor if wearing a brace or getting time off work is good for you.</li>
<li>For each pound you loose, you will reduce by the stabilization requirements on your MCL during squatting as if you had lost 4 to 5 pounds. Stabilizing a 200-pounds body-weight when squatting is equal in stabilization requirement on the MCL as if you were 1000-pounds and standing up on a flat surface.[4]</li>
</ul>
<h2>Things Not to Do If You Have a MCL injury</h2>
<p>The following exercises can further injure your MCL &amp; knee and should NOT be done until you are ready. Your sport physio can guide to know when you can restart these exercises.</p>
<p><strong>Do NOT do</strong> :</p>
<ul>
<li>Do not do squats past 90 degrees of knee flexion</li>
<li>Do not do high Impact and plyometric exercises</li>
<li>Do not do leg extension machine (quadriceps extensions)<br />

<h2>Complementary/Alternative Therapies</h2>

<li><strong>Prolotherapy</strong> with other products than ozone have sometimes been helpful[6], but there is not enough scientific support to recommend these therapy. Note that ozone therapy is some form of prolotherapy.</li>
<li><strong>Ozone therapy. </strong>Ozone therapy is one form of prolotherapy. It has very good anecdotal results in knee injuries, both on cartilage regeneration and tightening of ligaments. Finding a good therapist is however hard. Dr. Robert Rowen, MD and Terri Su, MD have a <a href="http://www.docrowen.com/" target="_blank" rel="noopener">good clinic</a> in California. If ozone fails, there is no harm (if done by a competent doctor) and one can still have surgery; but if you have surgery and it fails, the surgery may cause irreversible damage. Ozone therapy is also good for knee osteoarthritis[5].</li>
<li><strong>Advanced soft tissue release (ASTR)</strong>. Sometimes when recovering from injuries and surgeries scar tissues develop and merge various layers together causing pain during motion. Your healthcare provider should not offer these treatments on a fresh injury.<br />

<p>I was reading a medical article and found an error about creatinine levels in the <a href="https://en.wikipedia.org/wiki/Cerebral_vasculitis" target="_blank">Cerebral vasculitis</a> page.</p>
<p><a href="https://en.wikipedia.org/wiki/Cerebral_vasculitis" target="_blank">https://en.wikipedia.org/wiki/Cerebral_vasculitis</a></p>
<p>They mentioned <strong>urine</strong> creatinine above 1.5mg/dl as a diagnostic criteria. What?!? I can&#8217;t believe nobody spotted that in the four years it has been on the web (since 2005). Normal urine creatinine are more like 120mg/dl and abnormally high urine levels are above 250mg/dl. So 1.5mg/dl is an error and the 1.5 value reflects <strong>serum</strong> creatinine.</p>
<p>You can read in this paper that the actual criteria is 1.58mg/dl in the serum[1].</p>
